Production servers (rack optimised) are usually headless.
Production servers are also -usually- under strict access control.
Even if you disable C-A-D, there usually are other ways - like a power
switch, a power cable etc. that can be fiddled with to cause a
production outage. A decent datacenter will have protection against
all these.
